The guide at the bottom allows you to cross off when your charting is done!
CC - Chart Check
A - Assessment
P - Plan of Care
E - Education
O - Outcome
F - Falls
I/O - intake and outputs
Above the B/L/D - breakfast/lunch/dinner symbols is an area to chart glucose checks if they are needed and I will cross it off once I give insulin! There is a place for everything so you never forget to check important information!
**This product was created to be a helpful tool for notes. It should never replace checking the original data source. Due to varying environments and chemicals we cannot guarantee your notes will remain legible.**

I loved this item when I saw a nurse using it last year, and now that I am returning to bedside I had to get one. I think it takes a lot of alcohol wipes to get the pages clean, I will try a spray bottle of alcohol and cloth or paper towel next and see if that is faster. But regardless I still love it.

Love it, but over time the marker does seem to stay in some places. It’s light, but wish it removed all the way like it does when you first get it.
